Log in to My Neighborhood Team

As a client of a neighbourhood team in Utrecht, you have access to our online client portal My Neighbourhood Team. When you registered, we sent you an invitation for this by email. Did you not receive it? Contact the neighbourhood team in your district.

 
In My Neighborhood Team you can:
• view and update your plan
• keep track of appointments
• set goals and keep track of which goals you have achieved
• share documents with the neighborhood team employee who supports you
• view links and videos relevant to you 

Log in to My Neighborhood Team

 

Logging in for the first time

If you are logging in for the first time, you will need the email with login details. Also keep your mobile phone at hand. You will also need that to log in.

  1. Click on the link in the email you received to go to the login screen.
  2. Enter your username and password mail. You can find them in your email. The easiest way is to copy them from there. Then click on login.
  3. Change the received password to a password of your choice and click on login.
  4. You will now receive an SMS with a code on your mobile.
  5. Enter this code on the login screen and click on login. You are now in your personal My-Neighborhood Team environment.
